Request Medical Records
To request medical records online, you agree you are the patient or their immediate representative.
If you are any of the following:
- Legal service
- Lawyers office
- Record retrieval service
- Insurance company
You must call 209-664-5650 or fax 209-664-5657. Any requests made by anyone other than the patient or their representative will not be processed.
Request by Email, Fax, or Mail
Download and complete the Authorization Form. Send the completed Authorization Form along with copies of your government-issued photo ID and any supporting documents, e.g. power of attorney, advanced directive, conservatorship, to any of the following:

The Health Information Services Department is open 8:00 a.m. to 5:30 p.m., Monday through Friday and closed on observed holidays.
Complete and bring the Authorization Form, a copy of your valid government-issued photo ID, and any supporting documents to:
